Service Pathway

  • Our intake staff:

    • Greets youth, explains the hub, answers their questions, provides tour, and offers support options

    • Identifies the service the youth are seeking

    • Obtains youth name/pronouns and date of birth, introduces measurement-based care, and selects relevant tools for youth to complete

    • Youth take the tablet to the waiting area loaded with required forms (e.g. socio-demographic form, consent form, and clinical screening tools)

    • If youth need help answering the questions or using the tablet, they can get support from intake staff or a peer support worker, or wait until they see the service provider to finish completing the tools

    • Review consent, confidentiality, and expectations for session

    • Service provider and youth review results of clincal tools together and decide next steps

    • Service provider and youth co-create action plan in response to youth concerns

    • At the end of the session, youth are asked to rate their level of satisfaction and overall service experience

    • Discuss possible care options with youth and family/support individuals based on goals, risk, level of distress and functioning, clinical judgement, and youth preference

    • Service provider connects youth to appropriate Youth Wellness Team services and/or community and social support services
